Please bring a copy of your driver's license and insurance information.
We will file insurance for you, but ask that you cover the portion that insurance will not cover at the time of your visit. Most insurance plans have a co-payment, co-insurance and/or deductible. You are responsible for these payments. We will prepare an estimate of this cost and notify you a few days prior to your procedure. This payment is due at time of service and all charges incurred are for the facility.
You will receive a separate bill from the physician and from pathology if any biopsies are taken. If you are unable to pay on the date of the procedure, the appointment will be rescheduled.
For your convenience we will accept a personal check and all major credit cards.
