To ensure that your visit to our office is as convenient and efficient as possible, please fill out all of the forms listed below. You must print out all forms and bring them with you on the day of your procedure. If you are not taking any medicine you do not have to print out the medication list.
If you are a returning patient please fill out and sign all forms in the Patient Registration Packet. We will need to update/review you health information with each visit.Please bring your driver's license and insurance card. We will file insurance for you, but ask that you cover the portion that insurance will not cover at the time of your visit. Most insurance plans have a co-payment, co-insurance and/or deductible. You are responsible for these payments. We will prepare an estimate of this cost and notify you a few days prior to your procedure. This payment is due at time of service and all charges incurred are for the facility.
You will receive a separate bill from the physician and from pathology if any biopsies are taken. If you are unable to pay on the date of the procedure, the appointment will be rescheduled. For your convenience we will accept a personal check and all major credit cards.
